Evolution of the Management of AKI in Neonates

چکیده
We in Vicenza began studies on buffers in peritoneal dialysis (PD), leading the pathway toward the use of bicarbonate in fluids (1–3). We analyzed the pathophysiologic pathways of neurotransmission, measuring several substances in the cerebrospinal fluid before and after dialysis (4, 5). In those days, the pressure to shorten hemodialysis treatments provided the impetus for studies on dialysis tolerance. We therefore focused on developing technology to make short dialysis efficient and safe (6). We studied fluid mechanics and flow distribution in dialyzers and mechanisms of solute transport in hollow fibers, and we applied all of this information to support rapid hemodialysis (7) and continuous arteriovenous hemofiltration (CAVH) (8). We developed new filters (9), described the adsorption process onto the membrane, and made the first filters with two ports in the filtrate compartment so that we could perform the first hemodiafiltration treatment (10). At the same time, we developed new fluid balance devices to be applied in intensive care patients (11).

Predictive Factors of Acute Renal Failure in the Neonates with Respiratory Distress Syndrome
Background: Preterm birth occurs in a large number of pregnancies, and its incidence has been reported to be on the rise. Acute kidney injury (AKI) is a common complication in the premature infants with respiratory distress syndrome (RDS). The present study aimed to determine the predictive factors, clinical courses, and outcomes of AKI in the neonates with the clinical and radiological manifes...
متن کاملGlomerular Filtration Rate Changes in Neonates with Sepsis Treated with Amikacin and Cefotaxime
Background and purpose: Aminoglycosides (AG) such as Amikacin that are widely used to prevent and treat infections in neonates have nephrotoxic effects. The aim of this study was to evaluate the role of urinary index to Glomerular filtration rate (GFR) in diagnosis of tubular renal injury following administration of Amikacin in neonates admitted to neonatal intensive care unit (NICU). Materia...
متن کاملAcute Kidney Injury (AKI) in perinatal asphyxia
Background: Acute kidney injury (AKI) is a common consequence of perinatal asphyxia, occurring in upto 56% of these neonates. It is important to recognize AKI in asphyxiated neonates to facilitate administration of appropriate fluids and electrolytes in order to improve their outcome. Objectives: To determine the incidence of AKI in asphyxiated neonates and to correlate the severity and type of...
متن کاملPeritoneal dialysis in an extremely low-birth-weight infant with acute kidney injury
Critically ill neonates are at high risk for acute kidney injury (AKI). Renal supportive therapy (RST) can be an important tool for supporting critically ill neonates with AKI, particularly in cases of oliguria and fluid overload. There are few reports of RST for management of oligo-anuric AKI in the extremely low-birth-weight infant weighing <1000 g. We report successful provision of peritonea...

Abstact:objective To investigate acute kidney injury (AKI) in children with acute lymphoblastic leukemia (ALL) who received high dose methotrexate (MTX) chemotherapy and explore the corresponding treatment. Methods 180 children who received high dose MTX chemotherapy were observed with serum MTX concentration and serum creatinine. Patients with AKI of stage 3 or poor response to conventional tr...
متن کاملNeonatal Acute Kidney Injury.
In recent years, there have been significant advancements in our understanding of acute kidney injury (AKI) and its impact on outcomes across medicine. Research based on single-center cohorts suggests that neonatal AKI is very common and associated with poor outcomes. In this state-of-the-art review on neonatal AKI, we highlight the unique aspects of neonatal renal physiology, definition, risk ...
